After successfully changing his appearance, Chaz Bono is now asks judge to officially change his name and gender. Known as Chastity Bono, daughter of singer Cher, Chaz has reportedly filed a petition to change his name and gender last week.

According to news sources, Chaz Bono's doctor also filed a 'declaration' that the 41 year-old 'daughter' of Cher and Sonny Bono has performed a successful operation of changing his gender last year.
